For Medica Choice Care MSC+ (Minnesota Senior Care Plus)

This plan provides low or no-cost, high-quality health care coverage to adults age 65 and older who are eligible for Medical Assistance and live in the 50-county service area. It includes all Medical Assistance benefits, as well as Elderly Waiver services and care coordination (if you qualify). See Programs and Services below for more information.

This plan offers a Care Coordinator. These are nurses, social workers or highly trained professionals who become trusted partners in your health. Your Care Coordinator will work one-to-one with you. They will find and organize resources and providers to meet your health care needs and help you maintain a healthy, independent lifestyle.

Talk to a registered nurse any time of day or night with NurseLine™ by HealthAdvocate™. The nurses can answer your questions and provide guidance about your care options.





American Indians can continue or begin to use tribal and Indian Health Services (IHS) clinics. We won't require prior approval or impose any conditions for you to get services at these clinics. For elders age 65 years and older this includes Elderly Waiver (EW) services accessed through the tribe. If a doctor or other provider in a tribal or IHS clinic refers you to a provider in our network, you don't need to see your primary care provider before the referral.
