• Fully insured (PDF)
• Self-insured (PDF)

 

Not sure what kind of plan you have? Check your Medica ID card. If you have a self-insured plan, the back of the card will say "This plan is administered by Medica Self-Insured." If you don’t see this note, you have a fully insured plan.
