Special Needs Plans Model of Care

Special Needs Plans (SNPs) are a type of Medicare Advantage Plan that focus on certain vulnerable groups of Medicare beneficiaries. The Model of Care (MOC) is a document that is required by the Centers for Medicare and Medicaid Services (CMS) for health plans that offer a SNP. The MOC is a vital quality improvement tool and an integral component for ensuring members’ needs are identified by the SNP and addressed through the plan’s practices.


Required Model of Care Training

In accordance with CMS guidance, providers contracted to serve Medica’s SNP members must complete annual MOC training.

Medica Special Needs Plans (SNPs):

  • Medica DUAL Solution (HMO D-SNP) (MSHO)
  • Medica AccessAbility Solution® Enhanced (HMO D-SNP) (Integrated SNBC)

Training Details

  • Access the SNP MOC Training via Medica Provider College
  • Organizations are responsible for maintaining documentation of each provider’s completion of the training (e.g., dated training attendance list, dated electronic attestation, etc.) that can be provided to Medica upon request
  • Communicate completion of training to your organization’s representative to complete one Model of Care Training Attestation on behalf of the organization
  • Provider Model of Care Training Attestation can be found online either:
    • Go to www.Medica.com > Providers > Electronic Transactions > Provider Demographic update Online Tool (PDOT); or
    • Submit the form here.
