Pediatric Case Management Programs

Case managers are available to support pediatric members, newborn up to seventeen years of age, living with serious or complex health conditions, or experiencing an acute health event. Programs serving pediatric members include complex case management, transplant, transitions of care and pregnancy.

Our specialized case managers partner with families and caregivers to support pediatric members through coordination of medical, social, emotional, and developmental services and supports, facilitating access to comprehensive care to meet their unique needs. A personalized care plan is created based on mutual health goals and case managers provide ongoing management and support as the care team works toward those goals.
