HSA Plans without HSA Preventive Drug List
The cost information shown in this look-up tool is an estimate of copay and plan sponsor cost based on a common pharmacy plan design. The tool will give you a general idea of the potential costs of various medicines. Your actual pharmacy benefit will determine your costs. In addition, your plan may not cover all of the drugs shown in the look-up tool.
If your employer is based in Minnesota, North Dakota, South Dakota, Nebraska, Wisconsin or Harrison county, Mills county or Pottawattamie county in Iowa, and offers fully-insured Medica plans, or offers self-insured Medica plans with the $25 insulin cost relief program, you will pay no more than $25 for insulin per prescription unit. If your plan normally has a deductible for prescriptions, the deductible is waived for insulin.
